Anyone figured out PROGRAMMING FAILED

Hey everyone, just curious if anyone who had the problem of "programming failed" after a flash if they have a figured out what was wrong, if so could you help me out?

I can flash and flash and flash, proper banks and so on. Always get a programming failed, all my bios files came from XBINS.
and I know they are not corrupt.

I resolve a similar problem this evening in my Xbox 1.6 and xecuter 3, I think my cause a error in flashing process was by bus switch or own swith, if the other points like correct xbins donwloads, solder points are ok, try to sure the correct enable flash in board and switch!
