I would put it down to soldering, if you have a multimeter check for continuity with a probe on the underside of the motherboard and a probe on the qsb, most problems would be solder related

Is it possible that the Powercord is not connected? That would get you errors like that.

For nand-flashing you need the Powersupply to be connected, but the xbox mustn´t be turned on.

you do not need the av cable in and the xbox should have mains lead connected but not turned on (as stated above)
When using the multimeter put on probe on the pin header point on the underside of the motherboard and another on the corresponding point on the qsb, this helps to check if soldering is good, what wattage soldering iron did you use as it is possible to burn out the solder points if the soldering iron is to hot and you are not careful

Ok, alternative solder points is what I meant. If you lift, burn, wreck, ruin a point, the link I gave you gives an alternate soldering point for each. You just need to scroll down to the pic of the points you need, then probe with a ohm, multimeter for continuity from the alternative point to you pins on the qsb. Whatever points reading no continuity, you will need to run a jumper wire, or resolder them.
