Try this (not my work, taken from xbox-scene):
Console Reset Codes

Clear All Installed Game Updates and Console Cache
  1. Go to the "system" blade
  2. Select "memory"
  3. Press Y on the HD symbol
  4. Press X,X, Left Bumper, Right Bumper, X,X
  5. A message will appear saying: "Do you want to perform maintenance on your Xbox 360 storage devices?"
  6. Select Yes
Clear Any Failed system updates that cause the console to error.
  1. With the console off, press and hold the sync up button (the small white one)
  2. While holding the sync button press the power button to turn on the console
  3. Continue to hold the sync button until the Console has booted up completely.
  4. During the boot process the console should clear any failed updates, allowing you to use it normally.

Rename the system update folder to $$ystem Update. And if by chance u had a default xex that accompanied the update folder. Don't put it on the flash drive because it doesn't belong with the folder. Using it will give you e71 every time.

try this as i had similar issue:

make sure console is off
name the folder $$ystemupdate
put usb in console
power console on
just as the xbox logo finishes loading it should see the update

Try this, re-flash your first created 13599 freeboot image that is not patched with dashlaunch or anything. Next try the USB stick with 13599 dash on the root of the USB.

Report back ...

its because dash launch is blocking the xbox finding updates. just reflash dash launch2.23b and choose no when you get to block the xbox from finding sytem updates option. i set them up that way so they are not updated when they are sold to people who never had one before since one guy took it on himself to update one from a game disc. same reason you have parental lock on the console blocking live access. code for that is 0000.

you can use the dash launch portion of johnny2goods guide on updating a jtag to 13599 if your unsure hopw to flash dash launch.

whatever you already did to get the avatars working sort of is wrong so start over with a formatted drive too.

i have done that mate in launch.ini its got noupdater = false meaning it will find the update and i have format it before and it still did not pick the update up after format and reboot

it is that. the launch ini will make no odds to it no matter how you set the ini. i know cos i've just done it on that consoles twin brother i set up the same night to check it worked ok. you must be doing something wrong.
put this launch ini on your hard drive before flahing dash launch, edit your existing one do not create a new notepad doc.

[QuickLaunchButtons]
BUT_X = Hdd:\Content\0000000000000000\C0DE9999\00080000\C0DE99990F586558
Default = Hdd:\fsd2\default.xex
[Settings]
nxemini = true
pingpatch = true

run dash launch and it will find the ini and go through the setting with yes/no answers to add the rest in, choose no at the block updates bit and you should then only have to put the official update on a fat 32 pen stick to add the stuff you need.

i don't see what difference starting fresh with the nand would make but i did say format the hard drive before doing it as something in there is not right. up to you though, as long as your confident in writing the flash file ok it won't hurt to try it i guess.
another thing though, if your running fsd and the console is booting straight to fsd that won't allow the console to find the update either. it must boot from power off straight to nxe to pick up the update. try booting it holding rb on the controller with your usb and update plugged in to the console, if it still goes to fsd then delete fsd or rename it until you get past this.
