LITEON DG-16D4S - SLIM TX Replacement PCB Issue

Ran into an issue with my replacement PCB. I flashed it fine approx 6 days ago to LT 2.0. Worked great, could read originals and backups.

Yesterday, when I turned it on I get Unrecognized Disk (initially on a GOW3 backup that I had been playing the night before). I grabbed the original as well as a Dead Island Original to try with the same error.

I tried reflashing the drive with LT2.0 and the error continued. I then reflashed back to stock (0225) FW and was able to read the disks fine.

Did the Liteon Erase and reflashed to LT2.0 after that and got Unrecognized Disk again. Reflashed to Stock and got Unrecognized Disk as well.

Erased it again and flashed the stock again and the disks worked fine. I tried playing several games (GOW2&3, Dead Island, Mass Effect and Dragon Age 2) for a couple hours to see if it was maybe the laser acting up/dying but all the games played perfectly fine.

I then erased it a couple times again and tried flashing LT2.0 onto it and got Unrecognized Disk again. Flashed back to Stock and got Unrecognized Disk as well.

I have since that point, put the original PCB back in and it's working fine. I'd prefer not to request a replacement without making an honest effort to exhaust troubleshooting and it might just be a Noob mistake on my part.

Couple things to note...
1) Using X360USB and CK3 Pro. Works fine on other boxes and same FW files have been tested on a MCIX DVD without issue.

2) I am using the CK3 Probe Power Cable (without the Probe attached). Doubt that will have any effect, but...

3) When I first OUTRO the drive, it just sits there doing nothing at 'Sending Vendor Outro' (I have left it for up to 45 minutes) but if I click OUTRO again then it will answer with the Key Verified... Thanks C4E... every time after the first. Only does it with this PCB (first time I've used a replacement PCB so this might be normal).

I dont think the issue is to down to ur hardware.... I.e. The x360 or the ck3..... More than likely a dodgy pin on one of the chips.... If u have a jewelers 10x magnifier check the two main chips for any poor connections on the legs first

Also check the cables.... U know
Laser cables etc

Similar problem happened to me, where basically my drive would be detected as 02 5 and not 0225. Used to get open tray. It also worked for exactly 6 days lol.

Ended up buyin a new pcb, and hopefully receiving it tonight

Update: I borrowed a buddy's Lizard/Xtractor3 (following Diaboliq20's advice) to attempt the flash to rule out the X360/CK3 and got Unrecognizable Disk with both OFW and CFW as well. I tried each twice, with erasing the drive before hand and just straight flash.
