Why might a seemingly perfectly functional fresh install, after a perfectly smooth network flash, Frag on the next start? On Off On Off a couple times then Pffft.

Either wrong size bios flashed, dip settings wrong, or incorrect version bios version.
If you flashed an X3 with 1959 make sure the dips are on on off off.
if you flashed a 2.6 make sure you flashed a 512kb bios.

FRAG means the same thing 99.9% of the time
bad bios / bad install / user error
0.1% its faulty mod - however you already had flashbios booting so that cancels that out

Which bios are you using?
Open your xbox, wiggle your chip (take it out and put it in), and check the 5v connection.
